中美文化協會三十週年慶園遊會攤位申請辨法

1. The Cost of Booth for “2005 CACA 30^th Celebration Fun Fair” is $50.00 per booth
   每一攤位申請費為50美元 (Application Form)

2. 攤位排列順序以收件先後為準, 攤位費用請一次付清, 支票抬頭請寫 :  
   C.A.C.A. (Chinese American Cultural Association)

3. 申請截止日期2005年4月20日,請將費用與申請表寄到
   – P.O.Box 12, Edison, NJ 08818

4. 園遊會聯絡人: 韓偉 (732) 979-3125

5. 各攤位請自行準備所須用具及設備，並於結束後負責清理乾淨。

6. 園遊會期間，攤位申請人放棄任何法律告訴 CACA及所屬人員的權利。

Note:

• Please bring all necessary equipments to set up your own booth area (tent, tables, chairs, etc.)

• Please clean up the booth area after the event.

• All vendors will release CACA and its members from any liabilities including but not limited to personal injury and loss of properties associated with this event.

• Please make check payable to: CACA (Chinese American Cultural Association)

• Please mail check along with application to: P.O. Box12, Edison, NJ 08818
